Indigenes: N 14,000.00 Non-Indigenes: N 16,000.00 The breakdown includes a registration fee of N5,000.00 for indigenes and N7,000.00 for non-indigenes. Additional fees include a processing fee of N8,000.00, Nursing and Midwifery Committee fee of N500.00, and a customized file fee of N500.00.

- Educational Certificate – FSLC, WASSCE, GCE, NECO, or NABTEB
- Birth certificate or Age Declaration
- Certificate of Local Government of Origin
